This form is designed to be completed by those who are currently enrolled in our program and wish to extend that enrollment for another year. Once you have completed this form, please go to the Pay Online page to submit your re-enrollment fee.
If you are classified as Record Keeping (you purchase your own books), your re-enrollment fee will be $60. If you are Full Service (you purchase your books through the CHOICE office), your re-enrollment fee will be $25.
