please visit our Pay Online page to remit the enrollment fee. In addition, as a CLASS family, we will need a Pastoral Referral on file. Please click here to download that form.

    ENROLLMENT APPLICATION

    Please select the current school year. A school year runs from July through June. For instance, the 2015/2016 school year will include July 2015 through June 2016.

    Student Information:

    Please list any special circumstances we should be aware of, such as special needs.
    Please list any charges this student has faced, including but not limited to truancy, probation, misdemeanor, or felony. Be specific.

    If you do not know the school's fax number, simply leave this field blank.

    Family Information

    A parent or guardian must furnish the following information. If you receive your mail at a P.O. Box, please provide both your mailing address and your physical address. Thank you!
    Please state your relationship to the student (mother, father, step-parent, grandparent, etc.).
    Please indicate the level of education you have completed. Tennessee law requires parents conducting a home school to possess at least a high school diploma or a general education development certificate (GED). If you have not received a high school diploma or GED, please be prepared to have CHOICE staff contact you regarding a Proctor. This will need to be a person to whom you will legally assign responsibility for overseeing your child's homeschooling. Proctor paperwork must be completed in the CHOICE office by appointment.
    Please indicate whether this child or any of your other children have been homeschooled before.
    If you check an option other than "None of the above", please be specific about the situation in which your family is involved.
    If you have homeschooled any of your children before, please indicate when and where.
    Full Service is designed for those who have never homeschooled before or who would like extra help in obtaining curriculum. With this option, you would pay the CHOICE office to assign, purchase, and send curriculum to you. For your convenience, total fees may be paid in one lump sum or may be divided into affordable installments. If you choose to pay in one lump sum, you will receive a 5% discount on your total fees. If you choose to make installments, you must sign a Payment Agreement. A small convenience charge will be added to the total amount due on any payment plan. Record Keeping Only is designed for those who feel comfortable assigning and purchasing their own curriculum. You will be required to submit an Academic Plan, which lists the subjects, grade levels, and curriculum providers you wish to use. Our director will confirm that your choices meet state minimum standards, then you will be responsible to purchase your own curriculum.

    Parent Responsibilities

    CLASS is a full service homeschool program. We are committed to do our best to meet your needs in educating your child. However, parents are ultimately the primary educator. Please read the following information regarding “Parent Responsibilities” upon enrolling your student in CLASS.


    • The Parent/Teacher must have a High School Diploma or G.E.D. 
    • Parents must maintain current address and phone number information with the CLASS office.
    • The student's first set of books must be purchased within 30 days of enrollment.
    • Parents must keep assigned payment installments up to date in order to ensure their students have work at all times. A student who has books available to work on at all times will achieve his or her academic goals more consistently.
    • Parents should set goals with their students for study times, book work, and testing times. These goals should be reviewed and updated once per week.
    • Parents must keep up-to-date records of their child's attendance and test grades. Homeschool laws in the state of Tennessee require a minimum attendance of 180 days per grade level. One school day consists of at least 4 hours of study. CLASS will provide forms for documentation of attendance and grades.
    • Parents must remove final tests from books before allowing the student to begin working in the book. The final test must not be issued until the matching work book has been successfully completed. Test materials need to be kept in the supervision of a responsible parent and must be graded by a parent.
    • Students are required to test yearly, either the achievement test (proctored by our staff) or the ACT or SAT.
